A preliminary report released Wednesday found “serious conditions” at the Phoenix Veterans Affairs facility, including hundreds of veterans who were never placed on an official wait list and faulty scheduling practices that meant some veterans would never see a doctor.

“We identified an additional 1,700 veterans who were waiting for a primary care appointment but were not on the [electronic wait list,]” the report from the VA inspector general said. “Most importantly, these veterans were and continue to be at risk of being forgotten or lost in Phoenix [healthcare system’s] convoluted scheduling process. As a result, these veterans may never obtain a requested or required clinical appointment.”

The report verified that at least 40 veterans died while awaiting care on a secret wait list at the Arizona VA facility as a result of fabricated and manipulate government documents at the highest levels in the facility.

Currently 42 VA medical centers across the country have been identified as using the same deceptive practices resulting in untold numbers of Veterans not receiving needed healthcare. The number of Veterans who died as a result of intentional deception by the VA could number in thousands.

Manslaughter charges should be pursued by the DOJ for every VA employee involved and for those who knew of it and did nothing to stop the killings. Will Holder step up to the plate?

Whoever is in office should fix the problems of government as soon as it becomes clear. Everyone running for office says that they are going to make things right. Obama is on TV right now saying that he didn't hear about the problems at VA until just recently. If that is true, how come I've been hearing about the problems at VA for decades? Are our "leaders" telling me that those of us that served don't matter? I agree with Justme on this...some people should go to prison for causing the deaths of veterans. Once again Obama caught in big fat lie. He claims he didn't know about the deplorable conditions in the VA hospitals until he heard about it in the news two weeks ago? Obama allowed conditions and appointed leaders that allowed veterans to die rather than receiving medical care, he is just as guilty as anyone and should be held accountable for the deaths. His actions/inactions/cover ups/lies were nothing less than criminal.

Veterans Affairs Secretary Eric Shinseki apologized Friday for what he called "irresponsible and unacceptable" practices at VA health care facilities. "I cannot explain the lack of integrity amongof the leaders of our health care facilities," he said. "This is something I rarely encountered during 38 years in uniform.

Now comes Obamacare with leaders selected with the same qualities as those that led the VA.
